Enrollment begins with submitting an admission application agreement.

Prior to admission, all state forms must be filled out and returned and fees to be paid by parents/guardians. No child will be admitted to Kids Lab Child Care if any of the forms are incomplete.

• Admissions Application Agreement.
• Identification and Emergency Information - Child Care Centers (LIC 700).
• Child's Pre-admission Health History - Parents’ Report (LIC 702).
• Child's Pre-admission Health Evaluation - Physician's Report (LIC 701).
• Consent for Medical Treatment (LIC 627).
• Current Immunization Records
• Signed and dated receipt of Notification of Parents’ Rights (LIC 995).
• Personal Rights — Community Care Facilities, Child Care Facilities (LIC 613A) receipts, signed and dated.
• Parent Handbook Acknowledgement Form: Signed and dated.
• Topical Ointment Consent.
• Allergies noted.
• First month’s tuition.
• Extra clothing (1-2 outfits) in zip lock bag
• Filled water bottle-labeled
• Rest-time bedding (if full day)
• Diaper, wiper (if not potty training)
